Lumen Vitae Group connects families, individuals, and professionals to supportive housing, care, and community resources— all tailored to your unique needs through dedicated social work and public health professionals committed to empowering vulnerable populations.
Why Lumen Vitae Group Is Critically Needed
1
Geographic Barriers
Many areas in NY, NJ, and PA lack health professionals and reliable public transportation, creating significant access challenges.
2
Senior Care Gaps
Isolated seniors face difficulty accessing coordinated services, with existing support systems often sales-focused or overburdened.
3
Marginalized Populations
Vulnerable and criminalized communities frequently avoid state systems due to fear, leading to unaddressed needs and isolation.
4
Fragmented Services
Individuals experience "service fragmentation" where referrals lack follow-up, culturally competent care, and consistent communication.
📊 Lumen Vitae Group was created in direct response to widening service gaps, institutional barriers, and systemic inequities that impact access to care, wellness, and dignity, especially among vulnerable and underserved communities across New Jersey, New York, and Pennsylvania.
Geographic & Access Barriers (NY, NJ, PA)
According to the Rural Health Information Hub, many counties in NJ (e.g., Warren, Sussex), NY (e.g., Sullivan, Delaware), and PA (e.g., Bradford, Potter) qualify as Health Professional Shortage Areas (HPSAs), limiting access to care and support.
Senior & Disability Gaps
1 in 4 seniors in NJ, NY, and PA lives alone, and over 50% of older adults report difficulty accessing coordinated services (AARP, 2021).
⚠️ Marginalized & Criminalized Populations
BIPOC, immigrant, LGBTQIA+, and low-income individuals frequently avoid state systems due to fear of surveillance or retaliation (Urban Institute, 2020).
Service Fragmentation & Mistrust
Many individuals experience what is known as "service fragmentation"—where referrals are given but follow-up, access, or culturally competent care is missing (SAMHSA, 2022).
🧩 Why Lumen Vitae Group Fills the Gap
Lumen Vitae Group is not a replacement for clinical care—but a critical bridge between people and systems. Our services:
  • Provide non-clinical, trauma-informed resource navigation for those who mistrust or cannot access state programs
  • Center dignity, choice, and cultural responsiveness in every intake, referral, and consultation
